Is this for marking the patterns only or is it strong enough to notch through medium-weight layered fabric, too?

It's a tool more like a paper hole punch - designed to cut paper or tagboard patterns that you make/design where those diamond notches would appear on a pattern that you buy. You mark within the notch on your fabric with chalk or similar marking tool. I did try it on fabric when I got it. Didn't work and my Patternmaking instructor hissed at me for trying, saying that's not how it's used.

Does it cut the notch in fabric also

That's what is for. I would try no to cut paper with it or you'll dull it quicker. But I have been told that the way it's made it really doesn't matter. I didn't know anything about this tool until I delved into the Master Tailor world. Also the notch it makes helps keep your fabrics from fraying.
